    Learning Disability Housing Provision
 
2. What is the aim of your sub-group?
    To insure people with learning disabilities and their families have greater choice, control and access over where and how they live in Herefordshire.

4. When will your sub-group start meeting?
Previous group, but first meeting following the LDPB was held on 7 June 2010 with follow up meeting 18th August.
 
5. What do you hope your sub-group will achieve/
3 key priorities for this year are
 
1. Publish Housing Plan - Identifying needs and housing options to meet current and future needs of those with learning disabilities
2. Improved information for those with Learning Disabilities - seeking funding from the LDDF
3. Provide housing pathways of choice, through a range of housing options as identified in the housing plan needs assessment.
 
6. If your sub-group has started, what work has been done/what is planned for the future/
Existing group have been meeting regularly, to monitor the delevery of housing provision identified through the existing housing plan.
Planned for the future as above.
7. What do you hope the Partnership Board can achieve in Herefordshire?
Improved services and options to meet the needs of those with learning disabilities.

What is planned for the future?
 
we will look at ways to address the needs of people with learning disabilites when they go into hospital.
we will put special paperwork in place for the nurses to use which helps to show what is needed to support people better We will make sure everyone who goes to hospital has a "hospital passport"
